Special Information for Parents of School Age Children
Elementary, Jr. High and High school students planning to attend a Florida school in August 2009 must submit a Florida Certification of Immunization, DH 680 form, to their school before starting.
Students entering Kindergarten and 1st grade need documentation of 5 DTaP, 4 Polio, 4 HIB, 2 MMR (Measles, Mumps, Rubella), 3 Hepatitis B, 2 Varicella (chickenpox) and 4 PneumoConj vaccines.
Students entering 7th grade need documentation of a Tetanus Booster vaccine (TD/TDaP).

The Central Florida Immunization Coalition is a group of people representing health departments, physicians, hospitals, private healthcare providers, civic organizations, vaccine manufacturers, healthcare insurance companies, schools, parents, and other interested individuals and groups.
